This 12 month Subscription Includes:

• Primary Care Plus Physical Exam - 90 minutes to include complete Physical exam, conventional lab testing, screening tests &  referrals if needed
• Follow Up Primary Care Routine Visit - 30 minutes to review the conventional lab testing and complete additional screenings if needed
• Advanced biomarker testing may be ordered based on  individual need (this could include gut biome testing, adrenals, food  sensitivities, hormones, SIBO, full nutrient picture and toxic elements)
• Functional Medicine Testing & Treatment Plan Review 45-60 minutes
• 1-hour Virtual Nutrition Consultation
• 30-minute Follow Up Visit to be used either for Primary Care sick visit/urgent need OR follow up on Functional Medicine Treatment Plan
• Unlimited secure messaging
 
• Optional IV Nutrition Therapy
